A reconstruction of the breast with breast implants can be performed directly, under the same anaesthesia after removal of the mammary gland. The implant is inserted either in front of or behind the pectoral muscle. However, sufficient skin and soft tissue mantle must still be present. The method is not suitable for patients with large, contracted scar plates. This is often the case after irradiation. In some cases, a preliminary operation for skin extraction is necessary to prepare for the implantation, for which an expander is inserted.
